Financial Policy
Welcome to Purelivity Wellness. Our goal is to provide transparent pricing and a simple membership experience while delivering high-quality, evidence-based medical care. This Financial Policy explains your financial responsibilities when receiving services from Purelivity Wellness.
By scheduling an appointment, enrolling in a treatment program, or receiving services from Purelivity Wellness, you acknowledge that you have read, understand, and agree to this Financial Policy.
1. Self-Pay Practice
Purelivity Wellness is a self-pay medical practice and does not bill commercial insurance, Medicare, or Medicaid unless otherwise stated.
Patients are responsible for all applicable fees associated with their treatment programs.
Services may qualify for payment with a Health Savings Account (HSA) or Flexible Spending Account (FSA). Eligibility is determined by your plan administrator.
2. Testosterone Optimization Program
Patients interested in the Testosterone Optimization Program must complete an initial medical evaluation before treatment may begin.
The current initial consultation fee is $99 and includes:
- Comprehensive telehealth medical evaluation by a licensed healthcare provider
- Review of medical history and symptoms
- Initial laboratory testing ordered by the provider (laboratory costs included)
- Review of laboratory results
- Individualized treatment recommendations
The consultation fee is due at the time of scheduling and is non-refundable once the medical evaluation has been completed.
Completion of the consultation does not guarantee eligibility for testosterone replacement therapy or the issuance of a prescription. All treatment decisions are made solely at the discretion of the treating provider.
3. Medical Weight Loss Program
Medical Weight Loss patients are not charged a separate consultation fee.
Treatment begins with a medical evaluation conducted by a licensed healthcare provider to determine whether the program is medically appropriate.
Patients enrolled in the Medical Weight Loss Program pay the applicable monthly membership fee disclosed during enrollment.
Unless otherwise stated, the monthly membership includes:
- Medical evaluation
- Provider oversight
- Ongoing medical management
- Follow-up visits
- Prescription management
- Secure messaging
- Weight loss medication prescribed by your provider
- Care coordination
Laboratory testing may be required before or during treatment if deemed medically necessary by your provider.
4. Hair Restoration and Erectile Dysfunction Programs
Hair Restoration and Erectile Dysfunction treatments are available as optional services for eligible patients.
Existing patients enrolled in another Purelivity Wellness program generally do not require an additional consultation fee when, in the provider’s judgment, a separate comprehensive evaluation is unnecessary.
The applicable monthly program fee includes the medication prescribed by your provider unless otherwise disclosed during enrollment.
Patients who are not already established with Purelivity Wellness may be required to complete an initial medical evaluation before treatment can begin.
5. Monthly Memberships
Many Purelivity Wellness treatment programs operate on a recurring monthly membership basis.
Membership fees are disclosed before enrollment and generally include:
- Ongoing medical supervision
- Follow-up evaluations
- Provider oversight
- Treatment adjustments
- Prescription management
- Secure messaging
- Review of laboratory results
- Care coordination
- Program medication, when included in your enrolled treatment plan
Membership benefits vary by program and are explained during enrollment.
6. Automatic Billing
Memberships automatically renew each month until canceled.
By enrolling in a membership, you authorize Purelivity Wellness to charge your payment method on file for recurring membership fees and any other authorized charges related to your care.
Recurring payments are generally processed on or near your monthly renewal date.
You are responsible for maintaining a valid payment method.
7. Medication Fulfillment
Prescription medications are prescribed only when medically appropriate and at the sole discretion of your treating provider.
To improve convenience and reduce shipping costs, medications may be dispensed in quantities that differ from your monthly billing cycle, including up to a 90-day supply when clinically appropriate.
Your monthly membership fee compensates Purelivity Wellness for ongoing medical supervision, provider availability, prescription management, treatment oversight, follow-up care, laboratory review, and continued participation in your treatment program.
Receiving a larger quantity of medication at one time does not eliminate your obligation to continue paying monthly membership fees while your membership remains active.
8. Laboratory Testing
Routine laboratory monitoring is an essential component of many treatment programs and is required to help ensure the safe and effective use of prescription medications.
Testosterone Optimization Program
The initial laboratory testing required for enrollment in the Testosterone Optimization Program is included in the $99 initial consultation.
After enrollment, routine laboratory testing required for ongoing testosterone therapy monitoring is included as part of the monthly membership. Routine monitoring laboratories are performed at intervals determined by your treating provider based on your individual treatment plan and current standards of medical care.
If your provider determines that additional or specialized laboratory testing is medically necessary beyond routine monitoring, those tests may not be included in your membership. Any additional charges will be discussed with you before testing is ordered.
Medical Weight Loss, Hair Restoration, and Erectile Dysfunction Programs
Routine laboratory testing is not generally required for all patients enrolled in these programs. However, your provider may order laboratory testing before or during treatment if medically appropriate based on your medical history, symptoms, medications, or clinical findings. Unless otherwise stated as part of your treatment plan, any laboratory testing ordered for these programs is the patient’s financial responsibility.
Failure to complete required laboratory testing may result in delayed prescription renewals, temporary suspension of treatment, or discontinuation of therapy until the necessary monitoring has been completed.
9. Provider Discretion
All medical decisions remain solely within the professional judgment of the treating healthcare provider.
Payment of membership fees does not guarantee:
- Acceptance into a treatment program
- Prescription medication
- Medication refills
- Specific dosages
- Continued treatment
Providers may modify, discontinue, or decline treatment whenever medically appropriate.
10. Payment Authorization
By providing payment information, you authorize Purelivity Wellness to charge your payment method for:
- Consultation fees
- Monthly membership fees
- Authorized laboratory charges
- Outstanding balances
- Other charges authorized by you in connection with your treatment
You agree to keep your payment information current.
11. Failed or Declined Payments
If a payment cannot be processed:
- Membership benefits may be suspended.
- Prescription renewals may be delayed.
- Future appointments may be postponed.
- Outstanding balances remain due.
Purelivity Wellness may retry declined payments using the payment method on file.
Repeated payment failures may result in termination of membership.
12. Membership Cancellation
Memberships continue on a month-to-month basis until canceled.
To cancel, send a message through your patient portal at least 7 days before your renewal date.
Cancellation becomes effective at the end of the current billing cycle.
Membership fees already charged are non-refundable.
Cancellation does not eliminate responsibility for unpaid balances incurred before cancellation.
13. Missed Appointments
Patients are encouraged to provide at least 24 hours’ notice when canceling or rescheduling appointments.
Repeated missed appointments may result in dismissal from the practice.
Purelivity Wellness reserves the right to assess a missed appointment fee if disclosed at the time of scheduling.
14. Refund Policy
Medical services begin immediately upon enrollment and involve provider time, clinical review, and administrative resources.
Accordingly, Purelivity Wellness generally does not provide refunds for:
- Completed medical consultations
- Provider evaluations
- Membership fees already billed
- Completed telehealth visits
- Laboratory reviews
- Prescription management services
- Medications that have been prescribed or dispensed
Refunds may be issued only when required by law or at the sole discretion of Purelivity Wellness.
15. Prescription Medications
Federal and state regulations generally prohibit the return of prescription medications after they have been dispensed.
Purelivity Wellness cannot accept returned prescription medications or provide refunds for medications that have already been shipped or dispensed.
Questions regarding damaged shipments or medication defects should be directed to the dispensing pharmacy.
16. Shipping
Shipping times are estimates only.
Purelivity Wellness is not responsible for delays caused by:
- Pharmacy processing
- Medication shortages
- Weather
- Shipping carriers
- Incorrect shipping information provided by the patient
Shipping fees, when applicable, are generally non-refundable once an order has been processed.
17. Chargebacks
Patients agree to contact Purelivity Wellness regarding billing concerns before initiating a chargeback through their financial institution.
Initiating a chargeback does not eliminate a patient’s responsibility to pay for services that were provided.
Purelivity Wellness reserves the right to suspend or terminate membership and pursue collection of unpaid balances when appropriate.
18. Pricing Changes
Membership pricing may change from time to time.
Existing members will receive reasonable advance notice of any material pricing changes before the new pricing becomes effective.
Continued participation after the effective date constitutes acceptance of the updated pricing.
19. Collection of Unpaid Balances
Outstanding balances may be referred to a collection agency or legal counsel as permitted by applicable law.
Patients may be responsible for reasonable collection costs, court costs, and attorney’s fees where allowed by law.

21. Acknowledgment
By scheduling services, enrolling in a treatment program, or maintaining an active membership with Purelivity Wellness, you acknowledge that you have read, understood, and agree to this Financial Policy. You further acknowledge that all medical treatment and prescribing decisions are made solely by your licensed healthcare provider based on your individual medical needs and clinical judgment.
This Financial Policy should be read together with our Terms of Service, Privacy Policy, and Telehealth Consent.
