Month-to-month membership. Cancellation and refund terms are governed by the Purelivity Wellness Membership Agreement and Financial Policy. Charges for services already rendered, medications already ordered, dispensed, or shipped, or laboratory services already initiated are generally non-refundable unless otherwise required by law.

GLP-1 treatment isn’t appropriate for everyone. Your medical history is reviewed before medication is prescribed, and some conditions may mean treatment isn’t appropriate or requires additional review.
Treatment is not available through this program if any of these apply:
- Pregnancy, pregnancy planning and breastfeeding
- You are pregnant, trying to become pregnant, or breastfeeding.
- Certain thyroid cancer or endocrine conditions
- You or a close biological family member has been diagnosed with medullary thyroid carcinoma, or you have been diagnosed with Multiple Endocrine Neoplasia syndrome type 2 (MEN 2).
- Serious medication allergy
- You have had a serious allergic reaction to semaglutide, tirzepatide, or an ingredient in either medication.
- Certain digestive, gallbladder or kidney conditions
- A history of pancreatitis, significant gallbladder disease, chronic or significantly reduced kidney function, gastroparesis, bowel obstruction, or another severe gastrointestinal disorder.
Purelivity uses clinic-specific safety criteria that may be more restrictive than the medication’s official prescribing information.
Some health histories require individual clinical review and do not necessarily mean treatment is unavailable. A licensed provider reviews your complete health history before any prescription.
About compounded medications
When clinically appropriate, Purelivity Wellness may prescribe compounded semaglutide or compounded tirzepatide through a licensed compounding pharmacy. Compounded medications are not FDA-approved. The FDA does not review compounded medications for safety, effectiveness, or quality before they are marketed. Treatment is prescribed only after an individual medical evaluation and when the provider determines it is clinically appropriate.

Can I be treated if I’m pregnant, trying to become pregnant, or breastfeeding?
No. Treatment is not available through this program if you are pregnant, trying to become pregnant, or breastfeeding. If you are planning a pregnancy, say so before you start. Prescribing information for semaglutide instructs stopping at least 2 months beforehand; that interval is specific to semaglutide, and your provider will advise you if tirzepatide is prescribed.
I’m already taking a GLP-1 from another provider. Can I transfer?
Yes. Tell us during your screening. Your provider will review your history individually before any prescription is written, and transferring does not guarantee continuation of your current medication or dose.
How is the medication chosen?
Your provider decides whether medication is appropriate for you and, if so, which option makes sense after reviewing your health history, symptoms and goals.
Will I need lab work?
Routine laboratory testing isn’t generally required for every patient. If your provider determines testing is medically appropriate, those laboratory costs are your responsibility. If required testing isn’t completed, prescription renewal may be delayed or treatment may be paused or discontinued.
Do you take insurance?
Purelivity is a cash-pay practice. Services may qualify for payment with a Health Savings Account (HSA) or Flexible Spending Account (FSA). Eligibility is determined by your plan administrator.
Can I cancel?
Yes. The program is month-to-month with no long-term contract. To cancel, send a message through your patient portal at least 7 days before your renewal date. Fees already charged, and medications already ordered or shipped, are generally non-refundable.
