    Your provider reviews all of your laboratory results, then walks through them with you on a 30-minute video visit and discusses which treatment option — if any — is appropriate for you.

There is no membership fee on top, no separate laboratory bill, and no long-term contract. Optional add-ons exist for established patients, but nothing is added unless you ask for it and your provider determines it is appropriate. Completing an evaluation does not guarantee that any medication will be prescribed.

Your provider

Purelivity is intentionally built around provider continuity, so you'll know who is managing your care.

The provider who reviews your intake is the provider who reads your laboratory results and conducts your visits. You will not be handed off to a rotating panel of clinicians, and you will not have to re-explain your history to someone new.

That continuity is the point. Over time your provider gets to know your numbers, how you actually feel, and what you're working toward — which is what makes it possible to adjust your care thoughtfully instead of generically.

Purelivity offers both approaches because the right one depends on the person. Here is what each actually does.

Injectable Testosterone

$150per month, if prescribed

The most established approach to restoring testosterone to a healthy range.

Testosterone is supplied directly, by a small weekly injection you give yourself at home. Your provider adjusts the dose based on how you feel and what your follow-up labs show.

Testosterone therapy can suppress sperm production, so it is not appropriate for men with current or near-term fertility goals.

Enclomiphene

$150per month, if prescribed

An oral option that prompts your body to produce more of its own testosterone.

Rather than supplying testosterone from outside the body, enclomiphene signals your body's own hormonal pathway to increase production. It is taken as a capsule — no injections.

Because it works through your body's own production, enclomiphene is often considered for men who wish to preserve fertility. Your provider will discuss what that means for your situation.

You don't have to decide this.

Both options are legitimate medicine, and they are the same price. Which one is appropriate depends on your laboratory results, your symptoms, your medical history, and whether you may want children. Your provider will review all of it with you and recommend the right path — and will tell you honestly if neither is appropriate right now.

Enclomiphene is prescribed off-label for this purpose and is prepared by a licensed compounding pharmacy. Your provider will review the details with you.

Prescription treatment is provided only when medically appropriate following provider review. Not every option is appropriate for every patient.

Certain heart medications, including nitrates, may make tadalafil or sildenafil unsafe. Your provider will review your medications and cardiovascular history before prescribing.

Compounded medications are prepared by a licensed compounding pharmacy. Compounded medications are not FDA-approved, and the FDA does not review them for safety, effectiveness, or quality before they are marketed.

Add-on medications are intended to ship with your regular order whenever possible. Separate or expedited shipments may result in an additional charge.

What your plan includes.

When treatment is prescribed, your medication is dispensed by a licensed U.S. pharmacy and shipped to your home. Your selected TRT plan covers the medication and routine supplies that plan describes. Optional add-ons, any testing not included in your plan, and separately shipped items are priced separately when they apply.

What exactly does the $99 evaluation include?

Your provider's review of your medical history, the laboratory testing needed to reach a treatment decision, your choice of Quest Diagnostics or a home collection kit, a live telehealth visit with your provider, and a personalized treatment recommendation. There is no separate laboratory bill.

What happens if I don't need treatment?

Then we tell you. If your laboratory results and symptoms don't support treatment, your provider will explain what your results do show and discuss appropriate next steps. That is a legitimate and reasonably common outcome of a real evaluation — and it is the outcome an honest evaluation has to allow for.

Do I have to choose between testosterone and enclomiphene?

No. Both pathways cost the same, and your provider recommends the appropriate one after reviewing your laboratory results, symptoms, medical history, and whether you may want children. You are welcome to share a preference, and it will be part of the conversation.

How is enclomiphene different from testosterone therapy?

Testosterone therapy supplies testosterone directly to your body. Enclomiphene works differently: it is a prescription medication that acts on your body's own hormonal signalling to increase your natural testosterone production. Because of that difference, it is often considered for men who wish to preserve fertility. It is taken orally, prescribed off-label for this purpose, and prepared by a licensed compounding pharmacy. Your provider will discuss whether it is appropriate for you.

Why is a video visit required?

Testosterone is a Schedule III controlled medication. Federal and Texas requirements — and good medicine — call for a live evaluation with a licensed prescriber before it can be prescribed. We do not prescribe from a questionnaire.

Do you take insurance?

Purelivity is a cash-pay practice, which is what allows the pricing to stay flat and predictable. Your evaluation is $99 and ongoing care is $150 per month if treatment is appropriate.

Can I cancel?

Yes. Care is month-to-month with no long-term contract and no prepaid commitment.

Who can become a patient?

Adult men 18 and older who are physically located in Texas at the time of their telehealth visits. Your provider screens for conditions that make testosterone therapy unsafe during your evaluation.
