Certain heart medications, including nitrates, may make tadalafil or sildenafil unsafe. Your provider will review your medications and cardiovascular history before prescribing.
Compounded medications are prepared by a licensed compounding pharmacy. Compounded medications are not FDA-approved, and the FDA does not review them for safety, effectiveness, or quality before they are marketed.
Add-ons ship with your regular order whenever possible. A separate or expedited shipment may cost extra.
Testosterone pricing questions
Are there hidden fees?
No. It’s $99 to determine whether TRT is right for you, and if you proceed, $150/month covers your care. The only possible additional cost is testing or treatment beyond our standard protocol — and we’ll discuss it with you before anything is charged.
Do I pay for labs separately?
No. The laboratory testing needed during your evaluation is included in your evaluation fee. Once you begin treatment, your routine monitoring labs are included in your monthly TRT Care Plan.
What does the monthly plan include?
Your medication, monitoring labs, supplies, shipping, provider review of your labs, secure messaging, and dose adjustments when medically appropriate — in one flat price.
Does paying for an evaluation guarantee a prescription?
No. The evaluation determines whether testosterone therapy is appropriate for you. Sometimes it is; sometimes the honest answer is that it isn’t. Either way, you’ll leave knowing exactly where you stand.
When does the monthly plan begin?
Only after your provider reviews your evaluation and approves treatment. You’re never charged for a plan before there’s a treatment to manage.
Completing an evaluation and paying the applicable fee does not guarantee acceptance as a patient, a prescription, or continuation of any current testosterone regimen.
Medical weight loss

Month-to-month membership. Cancellation and refund terms are governed by the Purelivity Wellness Membership Agreement and Financial Policy. Charges for services already rendered, medications already ordered, dispensed, or shipped, or laboratory services already initiated are generally non-refundable unless otherwise required by law.
